The three continents that have been mentioned so far are Dheona, Ashural and Zephiran.
Dheona is the western continent, the one the campaign is set on. It includes all lands west of the Halueth river. I should start by explaining that the Halueth isn't actually a river, despite the name. It is the narrow body of salt water that separates Dheona from Ashural. At times the Halueth is only a few kilometers wide, but at times it expands to more than 200 kilometers. In places it is deep and in others it is shallow and marshy, filled with the Salt Fens. The Kith swamp folk are the only ones who know the secret ways of navigating through the fens and they control trade between the Sea of Endymion and the Bay of Cimber in the frozen north.
Ashural is the eastern continent. Significantly larger than Dheona, it stretches thousands of miles east from the Halueth river to the lands of the Dragon Emperor. In between lies the Sea of Grass, home to the nomadic Janthy (a tribe of mixed human and orc clans), the Sugda (human horse nomads), the Grassland Kith, the Plains Minotaurs and several other groups. Lugash, Deladdun, Eluquome, Sadiroth, Asticon, Khryseon and Karthis are all nations in the Sea of Endymion region that are on Ashural rather than Dheona.
Zephiran is the southern continent. Most people are most familiar with it as the home of the Rhodassans. It is said to contain a kingdom of serpentfolk, a land run by intelligent spiders, jungles full of lizardfolk and many other wonders.
